CHECK OUT PROCEDURE

Check out dates: June  9th

 

____1.  Check your grade book against the grading worksheets to make sure grades were entered each grading period.  There cannot be any I’s (Incompletes).  Teachers must sign and date each grade sheet and turn in to your teacher at check out. If you do not have a homeroom to check out, you will need to turn your grade book and signed grade sheets in to : Steve Morris.

 PLEASE PLACE NAME ON THE OUTSIDE OF GRADE BOOK.

 

____2.  Check the bottom of the report card for attendance (days present, absent and non-member).  This will have to be recorded on the permanent record folder.

 

____3.  Verify that the following information is on each permanent record folder:

·         Student’s full legal name

·         Birth date, place of birth and copy of birth certificate

·         Mother’s and Father’s name are listed on permanent record file as on the birth certificate.

·         Address and phone number listed

·         All grades accurately recorded (according to the label or if labels are unavailable according to the report card)

·         If using label, the MUST be cut to fit the space provided on the permanent record folder.  If not, they will have to be hand written.

·         Health card and Test card should be in permanent record folder

·         Entry date for this school year is 08-26-09 for any new students to our school and all 9th grade student.  For 12th grade students, you must record 05-27-10 and Graduated.

·         Watch for repeated classes.  (Mark thru with red ink pen)

 

·         Make sure that the number of credits listed on bottom of the transcript label is correct. (Credits for promotion are 6, 12, & 18)

·         All homeroom teachers will place the transcript label on the permanent record folder (cut to fit if necessary).  You must sign below the label and print the days present/absent, and non-member information that is on the bottom of the report card.

 

         4. Senior Homeroom Teachers Only:  Make sure that graduation information is completed.  (Cluster and pathway must be included in this information).

 

____5.  Department Heads are to place teachers within their department in teams of 2 for records check out. ( Teachers will review partner’s paperwork & initial check sheet.)

 

____6.  Collect Teacher Handbooks & return them to the office.  Please make sure that teachers name in located on the front inside cover.

 

____ 7.  Take textbook inventory for the department and place all textbooks in a central location.  Textbooks should be in stacks of 10.  Please let Mr. Swann know where the textbook are placed.  Dr. Smith will do a final count of all textbooks.

 

____8.  Cover computers in the classrooms, lock desks & cabinets, cover personal shelves for items that teachers do not want misplaced.

 

____9.  Clear out old papers, books, worksheets, etc…  Leave no extra materials for custodians to throw away or clean.

 

____10.  Teachers should place keys and badges in an envelope with their name and room numbers  on the outside and give them to Mr. Morris.

 

____11.  Please check with Jackie to pay lunch bill in full.

 

____12.  Check with Dixie to complete all financial & fundraiser profit/loss forms.

 

____13.  Coaches Only: All fall sport coaches must submit a list of potential player names for their perspective sport to Mr. Porter.
